Eligibility Children between the ages of 3 - 17 Parent(s) must be registered under the palliative program Family consents to referral
Referral Process Have your healthcare provider (nurse, social worker, physician) contact us at lastingmagic@hotmail.com We will receive the required information and contact you directly A visit will be made by one of our nursing members (either by phone call or home visit) We will discuss your needs/wishes and help you use your imagination We will contact appropriate resources and fulfill your wish
